Document Requirements for Enrollment

Proof of eligibility to work in the US
from the I-9 List of acceptable documents must be presented in person

Two reference letters
one professional and one personal, typed, dated and signed with a phone number

Proof of address
acceptable forms include a utility bill, rent or mortgage bill, cable bill, bank statement

Physical within the last year

4- 7 panel Drug Screen within the last 6 months

Proof of 2 MMR immunization Or Proof of immunity to Rubella, Rubeola, and Mumps

Flu shot from current flu season
(October - March)

Two-step TST/PPD Or IGRA QuantiFERON Gold or T-Spot, not older than 1 year

OR chest x-ray
not older than 10 years and poof of a previous positive test
