To start the enrollment process for a Home Health Aide, a member of our team will first reach out to complete an introductory call. Our helpful, experienced personnel are ready to answer any of your enrollment questions and to help explain more about the eligibility process.
Our Intake Coordinators will obtain referrals from your physician that will facilitate your insurance evaluation. We will be there for the client before, during and after the insurance evaluation. A representative can be sent to your home to help complete Medicaid’s confusing enrollment paperwork and forms, ensuring documents are submitted accurately & safely within the monthly deadline.
Once approved for Home Care Services, an authorization from Medicaid or the insurance plan is needed to start care. Our team will monitor your authorization status with Medicaid or the selected insurance plan, and communicate with you throughout. When the authorization is received, True Care Coordinator will reach out to discuss your caregiver and timeline for start of care.
A trained and qualified True Care caregiver will be assigned, along with a True Care Registered Nurse to create a plan of care that is personalized to the client for the caregiver to follow.
