Introductory Call
To start the enrollment process for a Private Duty Nurse, a member of our team will first reach out to complete an introductory call. Our helpful, experienced personnel are ready to answer any of your enrollment questions and to help explain the eligibility process.
Medicaid & Insurance Evaluations
Our Intake Coordinators will obtain referrals from your physician that will facilitate your insurance evaluation. We will be there for the client before, during and after the insurance evaluation. A representative can be sent to your home to help complete Medicaid’s confusing enrollment paperwork and forms, ensuring documents are submitted accurately & safely within the monthly deadline.
RN Skilled Assessment
Prior to a facility discharge, our clients receive a skilled assessment by a True Care Nurse to establish a personalized plan of care to be carried out at home.
In Home Visit
Our nursing staff prepares the client's home, orders all the necessary equipment, and ensures everything is working properly prior to the client's arrival.
Facility to Home
A True Care Nurse will accompany the client through the transition from facility to home care. We provide in-home care by monitoring the client’s medical status as well as perform skilled tasks in the comfort of their home.
