Working in conjunction with your physician, you have the ability to customize a care plan that is right for you. At True Care, we also understand that your insurance plan may not cover all of your needs. That’s why we offer private pay care so you or your loved one can supplement the care plan you are already receiving so that you may acquire the level of care you are most comfortable with.
True Care Solutions provides high-tech infusion and supplemental therapy services to care for a variety of medical conditions and ailments, including antibiotics, IVIG, TPN, high acuity, pain management, hydration, Remicade, chemotherapy, port, and PCA pumps. Our specially trained nurses can administer these solutions in the comfort of your home or educate a designated caregiver on how to administer them with side-by-side training. True Care will monitor your therapy closely, follow up, and manage your therapy schedules and infusion devices.
When you or your loved one has been diagnosed with a life-threatening illness, True Care is there to provide comfort and support for both the client as well as their family members. We provide the highest level of support by providing services like specialized care plans, comfort management, end-of-life support, spiritual counseling for clients. And for families: respite care, one-on-one meetings, supportive phone calls, grief counseling.
Providing individualized nursing care in the comfort of your home. A registered nurse is provided through Medicaid for those of all ages with long-term complex health conditions. Specifically: tracheostomy tube, gastrostomy tube, and ventilator-dependent clients. Our private duty nurses hold an LPN or RN license. They work one-on-one with families who require long-term skilled medical care. They work closely with the client’s family as well as their physician, acting as a liaison between all parties communicating and coordinating your plan of care in real-time. This program is available to any client who meets the aforementioned requirements and is currently receiving Medicaid benefits.
True Care’s Consumer Directed Personal Assistant Program, or CDPAP, allows you to hire the caregiver of their choice, including most family members and friends. At True Care, we know: “Care begins at home. Care begins with trust.” The ability to choose exactly who cares for you is not only helpful but empowering. We believe that individuals should have a say in the hiring and management of their well-being. CDPAP gives you the power to hire a close friend or relative (with the exception of a spouse or parent of a child under age 21) as your caregiver and pay them to provide this care. What’s more, personal assistants are not required to have special certification or licensing.
For clients with memory impairment, the answer is True Bridge. This specialized program helps bridge the gap between families and caregivers, with care that empowers, encourages, and engages our clients. Our innovative program includes activities intended to stimulate memory and rediscover purpose through activities such as re-exploring lifelong passions. These may involve music, arts and crafts, games, educational outings, and more.
The Waiver Program allows your loved one to stay at home with their family and caregivers instead of being moved to a nursing home or long-term care facility.
