True Bridge provides on-going caregiver training and support for caregivers and clients to connect in meaningful ways. Our True Bridge care starts with an Engagement Assessment visit, which highlights each client's strengths, interests, and abilities.
We provide tools to enrich the client's days and create connections with family and caregivers. Families are encouraged to participate and are an important part of the client's support circle.
