Your Home Care Service Options

If you are considering home care as a solution for either yourself or the care of a loved one, you likely have many questions about your service options. There is much to take into consideration when choosing the right care plan. Our staff is trained to customize and design a care plan that is perfect for our clients’ needs. Below are just a few examples of the types of service options best suited for the most common scenarios. Please be advised that these are general examples and, should you have questions regarding your specific case, you will need to contact our staff for more detailed assistance.

Short-term care
If an individual has Medicare, is considered home-bound, can leave the home only with assistance and/or great difficulty, and has a short term need for assistance they would be eligible for certain services. These services are almost always short term (exceptions include pediatric patients and patients using certain medical devices). The services would be provided by a certified home health agency (CHHA), like True Care, and would include some or all of the following: nursing visits, home health aide service hours, physical therapy, speech therapy, occupational therapy, dietitian and social worker visits. These visits may take place regularly for a time period of about 2 weeks up to 6 months. This would be appropriate for someone who was previously independent but recently had surgery or has a new diagnosis and needs support and education. Someone who had a recent fall due to increased weakness, who recently had a stroke or a joint replacement, or who has a wound and the individual, or family member, needs training/education to learn how to care for it. These are just a few examples of short-term cases where a client is eligible for home care services.

Long-term care
If an individual has Medicaid and has long term care needs they can be evaluated for home health aide services. The number of days/hours that would be authorized by the insurance company would be determined by the assessment. The hours/days can be appealed if the patient/family does not agree with the number of days/hours of home health aide services that were granted. The home health aide services also include periodic visits from a nurse. If an individual has Medicare and Medicaid they can receive both of the above services simultaneously if they are eligible.

Supplementary care
If an individual does not have Medicare or Medicaid, or if they feel they need more services than they are entitled to according to their insurance, they can pay out of pocket for additional services. These services would include nursing visits or home health aide services. The number of visits or hours provided would be determined by the individual. The nurse can provide education regarding diagnoses, medications or medical equipment. The nurse can provide IV infusion, oral or injected medications, wound care, percutaneous endoscopic gastrostomy (PEG) feeding, and many other duties. The home health aide can provide personal assistance, light housekeeping, and companionship.

Memory care
